In preparation for your telehealth appointment, please print out this eye chart to measure your acuity in each eye wearing your distance prescription of glasses or contacts. Tape on a wall 10 ft. away. Cover one eye and record the smallest line you can read with confidence.  Do the same for the other eye.  Record the fraction on the left of the chart as your acuity for that eye.
